Either keepassXC ( You can sync databases between devices with Syncthing if i remember right) But for a solution made to be hosted definitely BitWarden

Anything local (non cloud based) has a mininal chance of getting hacked. I can recommend KeePassXC or GNU pass. (if you like CLI programs)
